ACA Form 1095-B is used by health insurance carriers for employers with employer-sponsored group health plans, and for self-insured employers with fewer than 50 full time employees. ACA Form 1095-B (recipient copy) is used to report the months employees and family members were covered under the health plan; ACA Form 1095-BIRS (IRS copy) is used to report the months employees and family members were covered under the health plan; and, ACA Form 1094-BT is the summary transmittal record for form 1095-BIRS.
